Based on that statement it's obvious that Mr. Shapiro doesn't have a clue about what drives healthcare costs. Dishonesty and idiocy are what most right-wingers bring to discussions on healthcare. The profit motive associated with fee-for-service healthcare has been shown to drive up costs. Mr. Shapiro and his right-wing ilk are far too dishonest to aknowledge empirical truth aboiut healthcare.
